General Information for Mira Costa High School Athletics Department
Prospective athletes should express their interest in trying out by completing this form to ensure coaches have accurate contact information for each athlete. Athletes and their parents may be contacted prior to tryouts with information.
Prospective athletes must get a current sports physical signed by a physician and the form must be valid for 1 year. Even if you played a sport in the fall, you need to have a current physical on file that is valid through June 2026. It is a two step process:
1. Download the form
Please register for a SNAP! account prior to tryouts. If you played Indoor Volleyball or another sport earlier this year, log into your existing account and make sure the Girls Beach Volleyball check box is selected. Within the app, review and sign all legal forms:
Drug & Alcohol Policy, CIF Eligibility, Code of Conduct, Concussion protocol, COVID Guidance, Steroid Policy, Insurance Waiver, Sudden Cardiac Arrest Info, Travel & Treatment waiver.
NOTE: California Education Code states that all students who participate in a co-curricular activity must have insurance coverage prior to practice or participation.
For questions regarding the Physical Form, insurance, Snap! registration or related forms, please first visit the Mira Costa Athletic Packet site.
